Health Services

The MSD of Decatur Township is excited to begin the seventh year of our expanded partnership with Community Health Network. All Decatur school nurses are licensed health care professionals and are members of the Community Health Network Nursing Department. It is our goal to meet the health needs of all students and facilitate learning by keeping students in the classroom.

Immunization Requirements for 2018-2019 School Year

Get Vaccinated!

The full list of required immunizations for the 2018-2019 school year are provided in the link below. 

Please make special note of the following:

  • Kindergarten thru 4th grade students must have 2 Hep A, 2 MMR, and 2 Varicella (chicken pox) vaccines
  • 6th graders need 1 Tdap and 1 Meningitis vaccine and NEW this year 2 Hepatitis A vaccines 
  • Seniors need a Meningitis booster totaling 2 Meningitis vaccines and NEW this year 2 Hepatitis A vaccines
  • New for the 2018-2019 School Year from the Indiana State Department of Health:
    • 2 Hepatitis A are Required for grades K-4, Grade 6, and Seniors
    • Hepatitis A is recommended for students in grade 5, and grades 7-11 
    • Meningitis B is recommended for students in grade 12 

Please make sure your students are up to date on all immunizations before school begins. You may bring proof of immunization to registration or the first day of school. You can get these immunizations at your primary care physician office, Community Med Check, Community Clinic at Walgreens or the Marion County Health Department.
